Screen flickering DirectX 11

I mostly experience this problem with dgVoodoo 2 which is a DirectX 11 wrapper for older games but other Directx 11 applications like Heaven are also affected. Randomly the screen starts flickering and shifting like crazy (usually a few seconds after launching the application) similar to when you set a resolution or refreshrate your monitor doesn't support.

Even when I manage to close the game the same problem manifests itself on the desktop a few seconds later and the only way to fix it is doing a reboot.

That only seems to happen when the resolution is pretty low like 640x480 and the game is fullscreen but not in windowed mode. Other APIs are apparently fine.

I ran debugview to create a log and this message was pretty suspicious:

Error[PP_PowerPlay]: sislands_hwmgr.c[3328] -- Assertion '0' has failed: Unable to get std_vddc from CAC Leakage Table, using vddc.

It only shows up at low resolutions and seems to be related to voltage control.

Any ideas what's causing the issue?
